Doctor visit prep sheet

Choose the reason for your visit to get a one-page sheet: what to bring, what to note before you go, and questions to ask. Fill it in, print it or copy it.

Why are you visiting the doctor?

Pick a reason above and your sheet will appear here.

General prompts, not medical advice. The lists are general suggestions to help you prepare. They do not tell you what is wrong or what you need. Your doctor decides that. If you have an emergency, such as heavy bleeding, severe pain or a baby who moves less, call the hospital straight away instead of preparing a sheet.

How to use the sheet

  1. Choose the reason for your visit. The lists change to match it.
  2. Fill in the notes at home, when you have time. They are saved on this device so you can come back to them.
  3. Choose your 2 or 3 most important questions. The NHS suggests asking these first, so they are not left to the end.
  4. Print the sheet or save it as a PDF, or copy it as text to send to yourself. Bring it to the visit.

Good to know

  • Do not wait for an appointment if you have heavy bleeding, severe pain, leaking fluid, fever in pregnancy or reduced baby movements. Call the hospital on +91 944 103 0587, open 24/7 for delivery and emergency care, or see our emergency guide.
  • It is fine to ask the doctor to repeat or explain anything. Writing down what you are told, or asking someone to do it, helps.
  • Before you leave, check that you know what happens next, when, and who to contact if things get worse.
  • See also what is checked at each pregnancy visit.

  • Reason-specific prompts and the what-to-bring extras were written for this page as general suggestions. They are not taken from a single source and are not medical advice.

Frequently asked questions

Why prepare before seeing the doctor?

Appointments are short and it is easy to forget things. The NHS advises writing down your 2 or 3 most important questions, listing your medicines and noting when your symptoms started and what makes them better or worse. This sheet puts those steps in one place.

Is the list of questions medical advice?

No. The questions are general prompts to help you start the conversation. Your doctor will decide what applies to you, and you can cross out any that do not.

Can I print the sheet or save it as a PDF?

Yes. Press Print / Save as PDF and choose Save as PDF in your phone or computer’s print window. The website menu and footer are left out of the printout.

Is what I type saved or shared?

It is saved only in this browser on this device, so you can come back and finish it. It is never sent to us or anyone else. Press Clear to remove it.

What should I bring if I do not have old reports?

Bring what you have: your medicines, your notes and any prescriptions. It is fine to say you do not have earlier reports. Your doctor can advise which tests are needed.
